It would be great to have another option to refresh / reload the page by having the button directly on the toolbar. 

To visually demonstrate, it would be something like from the current state, i.e.,

to this one:

(edited as mockup idea)

This way, there can be another alternative aside from the toggle in settings saying "pull down to refresh page".

I second this.  The reload button went missing on the latest iteration of Firefox for Android, and this slows me down since I prefer "pull to refresh" to be disabled (I've had way too many experiences where I accidentally refreshed a page and lost what I was typing).  Now I have to tap the menu button at the top and go down somewhat to access refresh (or to long-press the back/forward buttons to skip to different points in the browsing history for that tab).
