Ability to add more doh providers (and automatic rotation)

At the moment, the screen where you can set custom DOH settings is quite basic. First of all, it could be improved by allowing users to enter many more providers. Secondly, it could offer the option of automatic provider rotation, e.g. changing the provider every day, every hour, etc.
