Currently the options (in Preferences) are

1) Preview in thunderbird
2) Always ask (which leads to an 'Open in...' or 'Save' submenu
3) Save file

Most of the time option 1 is fine. Occasionally I want to open in an external app, but the only way to do this (if Option 1 is selected) is to save it first - which I rarely want, and which adds unnecessary stuff to my PC.

So how about an option from the Previewer to 'Open in...' <default app> | <list of possible apps>?

Are you just talking about PDF files here?  I do not know of any other type of attachment where Thunderbird has a built-in reader, other than plain text files, and they just appear appended at the end of the message.

Yes, that was my specific use case.
It would apply more generally if other built-in readers exist.
