I have found that setting up the calendar notifications in the settings for Thunderbird does not work.
The fix came by clicking "Edit", then "Calendar Properties". From there is asks to set up notifications. I chose my preference of "2 hours before" and it worked.
I believe this is a programming fault which might be easily corrected.
