Thunderbird 115.8.0 (64-bit)
When I right-click on a jpg attachment and chose Open, I am taken to a Save dialogue. Didn't used to do this.
Irfanview is my default image viewer.

Try
Go to your profile
https://support.mozilla.org/en-US/kb/profiles-where-thunderbird-stores-user-data#w_how-do-i-find-my-... Open Folder
Then, shut down Thunderbird
Then, in your profile, delete the file handlers.json or rename the file (for example) handlers.json-old
Next, start Thunderbird
Next, go to Settings -> General -> Files and attachments and see what happens: the actions will be reset to new.
Next, perform your checks
handlers.json will be automatically recreated
For information purposes you can view the contents of the file, handlers.json, for example with Notepad
