Survey has mistakes in it, regarding the choice-options& I can't find a good way to report this tiny issue

I made this little survey by Firefox today & it had some issues regarding the choices one could take. (rather Disagree was twice there).

Also I couldn't find an easy way to give this feedback so I ended up here in the discussions - which might be worth a discussion itself.
